Certification (currently enrolled), signed by the Principal for Senior High School Graduating Students
Learner’s Progress Report Card for Out-of-School Youth
Transcript of Records (TOR) for Transferees
Signed application form (download via https://cats.scsc.edu.ph/)
Valid ID (preferably School ID)

Original copy of the following documents:

• Result of Entrance Examination and Interview (to be secured from the Guidance

and Counseling Office)

• Learner’s Progress Report Card (for freshmen only)

• Transcript of Records or TOR (for transferees only)

• Certificate of Transfer Credentials (for transferees only)

• Good Moral Character

• PSA Authenticated Birth Certificate

• Certificate of Residency from the Barangay

• Parents Income Tax (ITR/Certificate of Indigency/Certificate of Non-filing)

• Four (4)pcs. 2×2, three (3)pcs. passport size and three (3)pcs 1×1 ID pictures

(attire with collar; white background) taken within 6 months

• Police Clearance (for transferees only)

• Valid TESDA-issued National Certificate (NC)

• PSA Authenticated Marriage Certificate, for married women

𝐄𝐪𝐮𝐢𝐭𝐲 𝐓𝐚𝐫𝐠𝐞𝐭 𝐆𝐫𝐨𝐮𝐩𝐬 (𝐄𝐓𝐆𝐬) 𝐚𝐧𝐝 𝐑𝐞𝐪𝐮𝐢𝐫𝐞𝐝 𝐃𝐨𝐜𝐮𝐦𝐞𝐧𝐭𝐬:

1. Indigenous Peoples (IPs)- Certification from the NCIP or IPMR

2. First-Generation College Student in the Family- Certification from the Barangay or LGU

3. Person with Disability (PWD)- PWD ID issued by DSWD

4. Students from Geographically Isolated and Disadvantaged Areas (GIDA)-Certification from DILG or LGU

5. Solo Parent- DSWD-issued Solo Parent ID or Certification and photocopy of the child’s Birth Certificate

6. Child of a Solo Parent- DSWD Solo Parent ID or Certification and photocopy of Birth Certificate

7. Family of a Rebel Returnee- Certification from DSWD, AFP, or LGU handling reintegration programs

8. DSWD 4Ps Beneficiary- Certification from DSWD with Household ID Number

9. Working Student- Certification from employer or notarized affidavit (if self-employed/informal work)

10. Orphan- DSWD Certification and Death Certificate of parents or LGU Certification

11. Family of Subsistence Farmers or Fisherfolks- Certification from DA or BFAR, or BIR-certified ITR/Tax Exemption

12. Child of Person Deprived of Liberty (PDL)- Certification from BJMP or Bureau of Corrections and Birth Certificate (proof of relationship)
